Physical Therapist

Seeking a Full-Time Physical Therapist to provide personalized home health care. Key Responsibilities Include Delivering skilled physical therapy based on individualized treatment plans developed with physicians.Evaluating patient needs and establishing rehabilitation goals to enhance mobility, strength, and balance.Adjusting treatment techniques to optimize outcomes.Collaborating with interdisciplinary team members such as nurses, occupational and speech therapists, and social workers.Educating patients and caregivers on exercises, safety measures, and use of assistive devices.Documenting assessments, visits, and progress accurately and timely.Supporting patients in maximizing independence and quality of life at home. Qualifications Include Master’s degree in Physical Therapy from an accredited program.Current PT license or eligibility in the state.CPR certification.Strong communication, assessment, and problem-solving skills.Ability to work independently in home environments.Valid driver’s license, auto insurance, and reliable transportation. Preferred Experience (not Required) 1+ year in hospital, home health, or outpatient rehab settings.Experience with in-home therapy, geriatric, orthopedic, or neurologic rehab.Post-acute care or chronic condition management.Experience working with interdisciplinary care teams.
