About the Role
Provide personalized, in-home and community-based physical therapy care to older adults.
Responsibilities
- Perform evaluations, develop individualized treatment plans, and deliver evidence-based therapy to improve mobility, independence, and quality of life.
- Provide one-on-one therapy sessions in private residences, senior living, and memory care settings.
- Document patient progress accurately in electronic medical records, ensuring regulatory compliance.
- Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and fitness specialists.
- Educate patients, caregivers, and staff on safety, aging in place, and wellness strategies.
- Utilize telehealth and virtual care tools to enhance access and continuity of care.
Requirements
- Valid Physical Therapy license in applicable state(s).
- Degree from an accredited Physical Therapy program.
- Proficiency with electronic medical record (EMR) systems.
- Current CPR certification.
- Ability to meet physical demands, including lifting, repositioning, and extended walking.
