About the Role

We are seeking a licensed Physical Therapist to deliver one-on-one geriatric physical therapy in private residences and Senior Living Communities. This role focuses on enhancing mobility, independence, and quality of life for older adults through evidence-based treatment and personalized care.

Responsibilities

• Perform evaluations and develop individualized care plans for geriatric patients.
• Provide evidence-based physical therapy treatments to improve mobility and independence.
• Document patient progress and treatment details in EMR systems.
• Collaborate with interdisciplinary teams to coordinate care.
• Educate patients, caregivers, and staff on safety and aging-in-place strategies.
• Incorporate telehealth and virtual care for flexibility and continuity of treatment.

Requirements

• Valid Physical Therapist license.
• Degree from an accredited physical therapy program.
• Computer proficiency for EMR documentation.
• CPR certification.

Physical Demands

• Lifting and repositioning patients.
• Driving to patient locations.
