Position Overview

The Physical Therapist (PT) at Majestic Care of New Haven is responsible for evaluating, planning, and implementing individualized physical therapy treatments for residents in a skilled nursing facility (SNF). This role focuses on improving mobility, strength, endurance, balance, and overall functional independence to enhance residents’ quality of life. The PT collaborates closely with the interdisciplinary care team to deliver high-quality, resident-centered care.

Key Responsibilities

  • Conduct initial and ongoing evaluations to assess residents’ physical therapy needs, including mobility, strength, balance, and functional limitations.

  • Develop and implement individualized treatment plans based on clinical findings, resident goals, and functional abilities.

  • Provide direct physical therapy interventions such as therapeutic exercises, gait and balance training, mobility enhancement, and manual therapy.

  • Utilize evidence-based practices to promote optimal functional outcomes and independence.

  • Monitor and document residents’ progress, adjusting treatment plans as necessary to reflect changes in condition or goals.

  • Educate residents, families, and caregivers on treatment plans, home exercises, and safety strategies to support continued progress and prevent injury.

  • Collaborate with interdisciplinary team members—including nursing, occupational therapy, and speech therapy—to ensure coordinated, resident-centered care.

Documentation and Compliance

  • Maintain accurate and timely documentation of all evaluations, treatments, progress notes, and discharge planning in accordance with SNF, state, and federal guidelines.

  • Ensure compliance with all regulatory and facility policies, including HIPAA, OSHA, infection control protocols, and Medicare PDPM requirements.

  • Participate in quality improvement initiatives and contribute to the development of best practices within the rehabilitation department.

Professional Development and Education

  • Stay current with advancements in physical therapy, geriatric rehabilitation, and regulatory standards.

  • Engage in continuing education and professional development opportunities to enhance clinical skills and knowledge.

  • Provide training and mentorship to physical therapy assistants and other team members to support clinical excellence and consistency of care.

Supervisory Responsibilities

  • Oversee and support physical therapy assistants (PTAs) in accordance with state practice acts and facility guidelines.

  • Provide coaching, development, and corrective action as needed to ensure high-quality service delivery.

  • Evaluate team member performance and offer constructive feedback to promote professional growth and accountability.

Qualifications

  • Doctorate or Master’s degree in Physical Therapy from an accredited program.

  • Current state licensure as a Physical Therapist.
