Physical Therapist

About the role

Provide personalized, in-home care to older adults in private residences and senior living communities.

Responsibilities

• Deliver one-on-one therapy sessions focused on mobility, independence, and quality of life.
• Perform evaluations and develop individualized treatment plans using evidence-based techniques.
• Document patient progress accurately in electronic medical records.
• Collaborate with a multidisciplinary team including PTs, OTs, SLPs, and fitness specialists.
• Educate patients, caregivers, and staff on safety and aging-in-place strategies.
• Incorporate telehealth/virtual care to enhance access and flexibility.

Requirements

• A valid license.
• A degree from an accredited physical therapy program.
• Proficiency in electronic medical records (EMR).
• CPR certification.
• Meet physical demands such as lifting, assisting with transfers, and driving.
