About the role
Provide personalized, in-home care to older adults in private residences and senior living communities.
Responsibilities
- Deliver one-on-one therapy sessions focused on mobility, independence, and quality of life.
- Perform evaluations and develop individualized treatment plans using evidence-based techniques.
- Document patient progress accurately in electronic medical records.
- Collaborate with a multidisciplinary team including PTs, OTs, SLPs, and fitness specialists.
- Educate patients, caregivers, and staff on safety and aging-in-place strategies.
- Incorporate telehealth/virtual care to enhance access and flexibility.
Requirements
- A valid license.
- A degree from an accredited physical therapy program.
- Proficiency in electronic medical records (EMR).
- CPR certification.
- Meet physical demands such as lifting, assisting with transfers, and driving.
