Physical Therapist

Position Overview

The Physical Therapist (PT) contracted or employed through Healthy Living at Home - Arizona, LLC is responsible for providing physical therapy services and adhering to all conditions outlined in the Personnel Services Agreement.

Essential Job Functions

  • Provides physical therapy services to patients according to a written physician's plan of care.
  • This may include, but will not be limited to:
    • Assessing and evaluating therapeutic/rehabilitative/functional status.
    • Evaluating home environment.
    • Absessing for muscle strength, mobility, gait, ROM—potential for rehab.
    • Directing physical therapy treatment.
    • Instructing patients and families/caregivers in the use and care of therapeutic appliances.
    • Determining priority needs for physical therapy.
    • Reporting to physician patient’s reaction to treatment or changes in condition.
    • Initiating physical therapy program and instructing other personnel and/or family/caregiver members in certain phases of physical therapy.
    • Participating in case conferences.
    • May train patient in the use of prosthetic device.
    • Identifying patient and family/caregiver needs for other home health services and referring as necessary.
    • Preparing and submitting clinical and progress summaries based on the attainment of goals.
    • Participating in discharge planning for patient.
    • Preparing and submitting a clinical progress summary based on the attainment of goals as directed by Organization policy.
    • Providing physical therapy consultation to home families/caregivers when indicated.
    • Participating in in-service education programs for nursing organization personnel as needed.
    • Participating in peer consultation process.
    • Supervising Physical Therapy Assistants according to organization policy and state regulations.
    • When therapy is the only skilled service, instructing, supervising and evaluating home health aide care every two (2) weeks.
  • Other related duties as assigned.

Basic Qualifications

  • Master (MS) degree in physical therapy approved by an accredited organization.
  • Licensed to practice as physical therapist within the state, if applicable.
  • One (1) year or more experience as a physical therapist.
  • Good verbal, written communication and organization skills.
  • Possesses current CPR certification.
  • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

Physical Requirements

  • Typical functions of this role include standing, sitting and walking a majority of the time.
  • There are occasions when kneeling, stooping, bending or reaching may be necessary.
  • The employee must be able to lift and/or move up to 10 lbs on a regular basis.
  • There may be occasions in which the employee must be able to lift or move up to 40 lbs.
  • Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.
