Physical Therapist

About the role

We are seeking a licensed Physical Therapist to provide personalized, in-home, and virtual care to older adults in various settings, including private residences and senior living communities.

Responsibilities

• Conduct evaluations, develop individualized care plans, and deliver evidence-based therapy to support mobility, independence, and overall well-being.
• Document treatment progress accurately within the EMR system, complying with regulatory standards.
• Collaborate with an interdisciplinary team of healthcare professionals to ensure comprehensive patient care.
• Educate patients, caregivers, and staff on safety, function, and aging in place strategies.
• Incorporate telehealth and virtual care to enhance access and continuity of care.

Requirements

• Valid Physical Therapy license and degree from an accredited program.
• Proficiency in basic computer use and EMR systems.
• Current CPR certification and ability to meet physical demands such as lifting, repositioning, and assisting with transfers.
