We are seeking a licensed Physical Therapist to provide personalized, one-on-one care to older adults in private residences and senior living communities. The role emphasizes autonomy, meaningful patient relationships, and a community-focused approach that promotes aging in place. The position offers flexibility to incorporate telehealth and virtual care, allowing for greater access and continuity.

Responsibilities

• Perform evaluations and develop individualized care plans
• Deliver evidence-based treatments to support patient mobility, independence, and wellness
• Document patient progress and treatment details in EMR systems
• Conduct therapy sessions and provide patient education
• Collaborate with an interdisciplinary team of healthcare professionals

Requirements

• Valid Physical Therapy license
• Degree from an accredited Physical Therapy program
• Proficiency with EMR systems
• Current CPR certification

Physical demands

• Lifting and repositioning patients
• Driving to patient locations
• Wearing personal protective equipment (PPE)
