About the role
We are seeking a licensed Physical Therapist to provide one-on-one care to older adults in private residences and Senior Living Communities.
Responsibilities
- Performing evaluations
- Developing individualized care plans
- Delivering evidence-based therapy sessions to improve mobility, independence, and quality of life
- Effective documentation
- Collaboration with an interdisciplinary team
- Patient/caregiver education to promote safety and aging in place
- Incorporating telehealth and virtual care to enhance flexibility and continuity
Qualifications
- Valid PT license
- Degree from an accredited program
- Computer proficiency
- CPR certification
Physical demands
- Assisting with patient transfers
- Standing, walking, and driving
