About the role
Join our team as a Physical Therapist in a patient-centered environment dedicated to enhancing the lives of seniors.
Responsibilities
- Assess participants and develop individualized treatment plans.
- Provide therapy services, education, and training to participants, caregivers, and staff.
- Evaluate home safety, recommend equipment or modifications, and ensure safe mobility.
- Document assessments, treatment progress, and care plans in compliance with policies.
- Collaborate with an interdisciplinary team to create comprehensive care plans.
- Participate in team meetings, quality improvement initiatives, and organizational trainings.
- Train staff on safety, equipment use, and activity techniques to maximize independence.
- Maintain professional certifications, adhere to safety and confidentiality protocols, and contribute to a safe work environment.
Requirements
- Advanced degree in Physical Therapy and current state licensure.
- Experience working with elderly or frail populations preferred; training provided if needed.
- Valid driver’s license and auto insurance.
Qualifications
Experience in interdisciplinary team settings is advantageous.
