Role Overview Seeking a Full-Time Physical Therapist to deliver personalized home-based rehabilitation services. Responsibilities include evaluating patient needs, developing and adjusting treatment plans, collaborating with a multidisciplinary team, educating patients and caregivers, and documenting progress to enhance mobility, strength, and independence.
