About the role
We are seeking a licensed Physical Therapist to provide one-on-one care to older adults in private residences and senior living communities.
Responsibilities
- Perform evaluations and develop individualized treatment plans
- Deliver evidence-based therapy to improve mobility, independence, and quality of life
- Document care within the EMR system
- Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
- Educate patients, caregivers, and staff to promote safety and aging in place
Qualifications
- Valid Physical Therapist license
- Degree from an accredited Physical Therapy program
- Computer proficiency
- Current CPR certification
Schedule
Flexible schedule with opportunities for in-home, senior living, and virtual care. You will create and manage your own treatment schedule.
Physical demands
- Lifting and repositioning patients
- Driving to care locations
