Physical Therapist

About the role

The Gurwin Healthcare System is a full continuum of care offering a wide range of services including rehabilitation, skilled nursing care, memory care, respiratory and ventilator care, palliative and hospice care, adult day programs, on-site dialysis and infusion therapy. The position is available for the Certified Home Care Division.

Responsibilities

• Reports the patient's initial status, therapy plans and goals, on-going status and discharge plans.
• Provides assessments, plans and goals.
• Instructs patients, families and team members regarding programs that enhance and/or facilitate care and follow-up.
• Complies with all regulatory criteria.
• Provides professional leadership and clinical guidance and expertise to team members.
• Attends regular continuing education programs.
• Adheres to all aspects of Customer Service, including working together, showing empathy, courteous communication, respect of others.

Requirements

• Current New York State Physical Therapy License required.
• Graduate from a physical therapy program approved by the American Physical Therapy Association or the American Medical Association.
• A minimum of one year experience as a physical therapist.
• Ability to make clinical and managerial decisions within the scope of professional practice to perform physical therapy procedures competently and independently.

Qualifications

• High school diploma or equivalent.
• Completion of an accredited physical therapy program.
• Passing scores on the national licensure examination.

• Proficient in Microsoft Office.

Schedule

Hourly rate is pending credentialing. Position covers Stony Brook to Riverhead and other towns as needed.
