Sterling County Nursing Home, a provider of short- and long-term nursing and rehabilitation services, is seeking a Physical Therapist PRN to join our team in Sterling City, Texas.
About the role
As a Physical Therapist PRN, you will develop and implement personalized Physical Therapy plans of care for facility residents and short-term patients, while providing supervision and training to staff Physical Therapy Assistants.
Responsibilities
- Develop and implement the Physical Therapy plan of care for facility residents and short-term patients
- Provide supervision, as needed, for staff Physical Therapy Assistants in delivery of the PT plans of care
- Assist as needed with patient screenings for the need for Physical Therapy
- Communicate progress and any barriers toward PT goals with interdisciplinary team members
- Complete timely PT documentation in compliance with regulatory, licensing, and payor requirements
- Assist with maintaining both the rehab gym equipment and treatment area in a safe manner, using good infection prevention methods
- Provide and document patient, family, and caregiver training regarding each PT patient’s specific plan of care and recommendations for follow-up
Requirements
- Current Texas licensure as Physical Therapist
- Current CPR certification preferred
- Ability to reliably commute or plan to relocate to Sterling City, TX 76951 before starting work
