About the role
We are seeking a licensed Physical Therapist to provide one-on-one care to older adults in private residences and senior living communities.
Responsibilities
- Perform evaluations and develop personalized treatment plans
- Deliver evidence-based therapy sessions to enhance mobility, independence, and wellness
- Document care within electronic medical records
- Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
- Educate patients, caregivers, and staff to promote safety and aging in place

Requirements
- Valid Physical Therapist license
- Degree from an accredited Physical Therapy program
- Basic computer skills
- CPR certification
- Ability to meet physical demands such as lifting and assisting patients
