Role Overview

A licensed Physical Therapist providing personalized, in-home and senior living care to older adults, focusing on improving mobility, independence, and quality of life through evidence-based treatment within a flexible, patient-centered model.

Responsibilities

• Deliver one-on-one physical therapy sessions in private residences and senior living communities.
• Perform evaluations, develop individualized care plans, and implement evidence-based treatments.
• Document progress accurately in electronic medical records, adhering to regulatory standards.
• Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and fitness specialists.
• Educate patients, caregivers, and staff on safety and aging-in-place strategies.
• Incorporate telehealth and virtual care to extend access and flexibility.

Qualifications & Skills

• Valid Physical Therapy license or eligibility in the practice state.
• Degree from an accredited PT program.
• Proficiency with computers and electronic medical records.
• Current CPR certification.
• Physical ability to perform patient transfers, repositioning, and prolonged standing/walking.
