Role Overview
A licensed Physical Therapist providing personalized, in-home and senior living care to older adults, focusing on improving mobility, independence, and quality of life through evidence-based treatment within a flexible, patient-centered model.
Responsibilities
- Deliver one-on-one physical therapy sessions in private residences and senior living communities.
- Perform evaluations, develop individualized care plans, and implement evidence-based treatments.
- Document progress accurately in electronic medical records, adhering to regulatory standards.
- Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and fitness specialists.
- Educate patients, caregivers, and staff on safety and aging-in-place strategies.
- Incorporate telehealth and virtual care to extend access and flexibility.
Qualifications & Skills
- Valid Physical Therapy license or eligibility in the practice state.
- Degree from an accredited PT program.
- Proficiency with computers and electronic medical records.
- Current CPR certification.
- Physical ability to perform patient transfers, repositioning, and prolonged standing/walking.
