Physical Therapist

We are seeking a licensed Physical Therapist to deliver one-on-one care to older adults in private residences and senior living communities, using our Geriatric House Calls™ model. Key responsibilities include conducting evaluations, developing individualized treatment plans, providing evidence-based therapy to improve mobility, independence, and quality of life. The role involves effective documentation, collaboration with an interdisciplinary team, and patient/caregiver education to promote safety and aging in place. Clinicians may incorporate telehealth and virtual care to enhance flexibility and care continuity. Qualifications include a valid PT license, degree from an accredited program, computer proficiency, and CPR certification. Physical demands include lifting and patient transfer assistance.
