About the role

Provides comprehensive physical therapy services to patients in their homes, including assessment, developing, and implementing personalized care plans. Works under general supervision, assessing patient needs, monitoring progress, and adjusting treatment plans as necessary.

Counsels caregivers and families, provides education, and assists with therapeutic interventions, including positioning, transfers, and use of medical equipment such as wheelchairs and orthoses. Travels to patient locations, managing supplies and technology devices, ensuring infection control, and adhering to agency standards.

Evaluates equipment needs, manages caseloads, and collaborates with interdisciplinary teams. Serves as a Coordinator of Care for select cases and participates in meetings and special projects.

Requirements

  • NY State License and registration in Physical Therapy
  • Bachelor’s Degree in Physical Therapy
  • Minimum one year of experience or successful probationary completion as a VNS Health Rehab Intern
  • Basic PC skills; bilingual skills may be required based on operational needs
