Physical Therapist

About the role

We are seeking a licensed Physical Therapist to deliver one-on-one in-home and community-based care to older adults using our Geriatric House Calls™ model.

Responsibilities

• Perform evaluations and develop individualized treatment plans
• Provide evidence-based therapy to enhance mobility, independence, and quality of life
• Document care within our EMR system
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
• Educate patients and caregivers
• Incorporate telehealth for increased flexibility

Requirements

• Valid Physical Therapist license
• Degree from an accredited Physical Therapy program
• Proficiency with EMR systems
• Current CPR certification

Physical demands

• Assist patients with transfers and repositioning
• Extended walking or climbing stairs
