We are currently recruiting for a compassionate Physical Therapist (PT) to reliably serve adult and geriatric home health patients throughout Gloucester County.

Responsibilities
- Perform home visits in designated geographic territories
- Complete comprehensive physical therapy evaluations
- Assess mobility, strength, balance, gait, transfers, endurance, and safety risks
- Develop individualized treatment plans to improve function and independence
- Provide therapeutic exercise, gait training, balance training, and other evidence-based interventions
- Collaborate with nurses and other clinicians as part of a multidisciplinary care team
- Educate patients and caregivers on exercises, safety, and disease management
- Document patient care using a user-friendly electronic medical record system
Requirements
- Active and current license to practice as a professional Physical Therapist (PT) in the state of NJ
- Previous home healthcare experience welcomed, but not required
- Ability to travel to cases as assigned
- A calm and reassuring presence, active listening skills, excellent assessment skills, and strong written and verbal communication
- The ability to deliver patient care as you would want those you love to be treated, with compassion, excellence, and reliability

Pay
Base Pay, depending upon qualifications and subject to negotiation: $60.00 - $70.00 per point ($93,600 - $109,200 annual earning potential).
