Physical Therapist

About the Role

Full-time Physical Therapist providing home health visits in designated areas.

Responsibilities

• Conduct home visits to assess clients' mobility, strength, balance, and safety.
• Implement wound care interventions using evidence-based methods.
• Develop, execute, and revise individualized care plans including manual exercises, gait training, and balance activities.
• Collaborate with multidisciplinary teams to ensure comprehensive care.
• Educate clients and families on rehabilitative activities to promote safety and independence.
• Accurately document observations, interventions, and evaluations via electronic records.

Requirements

• Active New Jersey Physical Therapist license.
• At least one year of recent PT experience.
• Graduation from an accredited physical therapy program.
• Ability to work independently, manage time, and problem-solve effectively.
• Strong interpersonal and organizational skills.
• Proficiency with computers; EMR experience preferred.
