Physical Therapist

About the role

Plan, organize, and deliver Physical Therapy services for participants, including assessments, treatment, and education for participants, caregivers, and families to support independence in the community. Provide high-quality care as part of the interdisciplinary team (IDT).

Responsibilities

• Conduct assessments at enrollment and as needed, evaluate equipment and home safety, and develop treatment plans.
• Administer therapy, document progress, and adjust plans accordingly.
• Educate participants and train staff in safety, transfers, and mobility techniques.
• Collaborate with team members to create comprehensive care plans and participate in team meetings.
• Maintain accurate documentation, adhere to policies, and comply with safety guidelines.
• Participate in quality improvement, ongoing education, and community meetings.

Requirements

• Advanced degree in Physical Therapy from an accredited institution.
• Current state license to practice.
• Valid driver’s license and auto insurance.

Qualifications

• Preferably 2+ years of experience working in interdisciplinary settings and with elderly populations.
• Strong communication, teamwork, and documentation skills.
