Physical Therapist

Role: Physical Therapist Responsibilities Provide one-on-one physical therapy to older adults in their homes or senior living communities.Perform evaluations, develop individualized treatment plans, and deliver evidence-based therapies to enhance mobility, independence, and wellness.Document patient progress accurately within electronic medical records, adhering to regulatory standards.Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and Fitness Specialists.Educate patients, caregivers, and staff on safety, function, and aging-in-place strategies.Incorporate telehealth and virtual care to increase access and flexibility when appropriate. Qualifications & Skills Valid Physical Therapy license or eligibility in the practice state.Degree from an accredited Physical Therapy program.Proficiency in computer and EMR systems.Current CPR certification.Physical ability to perform transfers, reposition patients, and carry out therapeutic activities, including lifting and mobility tasks.
