About the Role
Provide personalized, in-home and senior living care to older adults as a Licensed Physical Therapist.
Responsibilities
- Conduct evaluations and develop individualized care plans.
- Deliver evidence-based therapy to support patient mobility, independence, and wellness.
- Document accurately in the electronic medical record (EMR).
- Collaborate with interdisciplinary teams to coordinate care.
- Educate patients and caregivers on therapy goals and techniques.
- Incorporate telehealth when appropriate.
Requirements
- Valid Physical Therapist license.
- Degree from an accredited physical therapy program.
- Computer proficiency for EMR documentation.
- Current CPR certification.
- Ability to meet physical demands, including lifting and repositioning patients, driving, and wearing PPE.
