About the role
Grandview Nursing and Rehabilitation Center is seeking a Full-Time Physical Therapist (PT) to join their rehab team. The position is open to both experienced clinicians and new graduates.
Responsibilities
- Evaluate and treat residents to improve mobility, strength, balance, and functional independence
- Develop and implement individualized, goal-driven treatment plans
- Document evaluations, treatments, and progress using Net Health and PointClickCare
- Collaborate with an interdisciplinary team to support resident-centered care plans
- Provide education to residents, families, and staff as appropriate
Requirements
- Active Pennsylvania Physical Therapist (PT) license, or eligibility to obtain
- SNF/LTC experience preferred, but new graduates are welcome and encouraged to apply
- Familiarity with Net Health and PointClickCare is a plus
- Strong clinical judgment, communication, and teamwork skills

- Passion for patient-centered rehabilitation and helping residents achieve their highest level of independence
