This role involves providing personalized one-on-one physical therapy to older adults in private residences and senior living communities, supporting mobility, independence, and quality of life through a flexible, patient-centered approach.
Responsibilities
- Perform evaluations, develop individualized treatment plans, and deliver evidence-based therapy.
- Support patient wellness, mobility, and functional independence through therapy sessions.
- Document care effectively within the EMR system.
- Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and Fitness Specialists.
- Educate patients, caregivers, and staff to promote safety and aging in place.
- Utilize telehealth/virtual care to enhance access and continuity.
Requirements
- Valid Physical Therapy license or eligibility in the state.
- Degree from an accredited Physical Therapy program.
- Proficiency in basic computer and EMR systems.
- Current CPR certification.
Physical Requirements
- Ability to perform moderate physical effort, assist with patient transfers, repositioning, and navigate diverse environments, including driving to patient locations.
