About the Role
A licensed Physical Therapist provides one-on-one in-home and senior living community care to older adults, enhancing mobility, independence, and quality of life through evidence-based treatments within a flexible, patient-centered model.
Responsibilities
- Conduct evaluations and develop personalized care plans
- Deliver therapy sessions supporting wellness, mobility, and functional independence
- Document care accurately in electronic medical records
- Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and fitness specialists
- Educate patients, caregivers, and staff on safety and aging-in-place strategies
- Utilize telehealth and virtual care to extend access and flexibility
Requirements
- Valid state licensure or eligibility in Physical Therapy
- Degree from accredited PT program
- Proficiency in basic computer and EMR systems
- Current CPR certification
- Physical ability to perform patient transfers, repositioning, and mobility tasks
