Physical Therapist

About the Role

A licensed Physical Therapist provides one-on-one in-home and senior living community care to older adults, enhancing mobility, independence, and quality of life through evidence-based treatments within a flexible, patient-centered model.

Responsibilities

• Conduct evaluations and develop personalized care plans
• Deliver therapy sessions supporting wellness, mobility, and functional independence
• Document care accurately in electronic medical records
• Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and fitness specialists
• Educate patients, caregivers, and staff on safety and aging-in-place strategies
• Utilize telehealth and virtual care to extend access and flexibility

Requirements

• Valid state licensure or eligibility in Physical Therapy
• Degree from accredited PT program
• Proficiency in basic computer and EMR systems
• Current CPR certification
• Physical ability to perform patient transfers, repositioning, and mobility tasks
