Physical Therapist Assistant (PTA): In-Home Outpatient | Per Diem

Per diem W‑2 position serving Nassau County (Baldwin, Freeport, Oceanside, Rockville Centre). Anticipated start Q1 2027; interviewing now for January starts.

About the role

• Local focus: Serve Baldwin, Freeport, Oceanside, and Rockville Centre, with visits grouped geographically whenever possible.
• Meaningful care: Build ongoing relationships with clients through one‑on‑one visits, generally planned for approximately 60 minutes, connecting treatment to their goals and daily life.
• Practical support: FFITT coordinates intake and billing, supports scheduling, and provides access to the supervising PT for clinical questions and care planning.
• Supported start: Receive paid initial training and guidance on clinical and administrative workflows.

Responsibilities

• Provide care under a physical therapist’s established plan of care.
• Educate clients and caregivers on exercises, mobility, and home strategies.
• Collaborate with the supervising PT to refine treatment strategies, support progress, and address concerns or changes in the client’s condition.
• Complete timely, compliant documentation using our EMR, with templates and workflow guidance to support your work.

Qualifications

• Current New York State PTA licensure and registration.
• Current CPR/BLS certification.
• Minimum of two years of direct clinical experience as a PTA by the start date.
• Strong clinical judgment, communication, and follow‑through.
• Comfort providing care in clients’ homes with PT supervision and team support.
• Reliable transportation within the service area.
• Experience with older adults or home‑based care preferred.

Pay

$55 per completed visit, plus paid initial training. No separate mileage or gas reimbursement. Full compensation terms will be provided in writing before accepting an offer.

Schedule

Scheduling is coordinated around your availability and patient needs. Initial caseloads will build gradually; visit volume is not guaranteed.

We are interviewing now for anticipated starts beginning in January 2027, with additional starts throughout the first quarter. Your start date and onboarding schedule will be coordinated around patient demand and mutual availability.
