Physical Therapist Assistant PTA Home Health PRN

About the Role

Work one‑on‑one with patients in their homes, helping them gain strength and independence to participate in daily activities such as attending church or gardening.

Responsibilities

• Provide physical therapy assistance directly in patients’ homes.
• Assess patient needs and tailor interventions to promote functional independence.
• Collaborate with physical therapists and other care team members to develop and adjust care plans.
• Educate patients and families on exercises, safety, and equipment use.
• Document patient progress and communicate updates to the clinical team.

Minimum Qualifications

• Bachelor’s or Associate degree from an accredited Physical Therapist Assistant program.
• Current state license to practice as a PTA.
• Current CPR certification, negative TB screen, and Hepatitis consent/declination.
• At least one year of experience in a home health setting.
