Responsibilities
- Deliver High-Quality Patient Care
- Provide direct physical therapy services under the direction of a licensed Physical Therapist
- Follow individualized plans of care to help patients reduce pain, improve mobility, and restore function
- Assist patients with therapeutic exercises, gait training, balance, and functional mobility
- Support patients with transfers, strengthening, and activities of daily living
- Utilize therapeutic modalities such as heat, cold, ultrasound, and electrical stimulation as appropriate
- Educate patients and caregivers on exercises, safety, and use of assistive devices
- Monitor Progress & Communicate
- Observe and report patient progress, response to treatment, and any changes in condition
- Maintain accurate, timely documentation in the EMR
- Communicate effectively with the Physical Therapist and interdisciplinary care team
- Participate in case conferences and care coordination efforts
- Collaborate & Contribute
- Work closely with nurses, therapists, and other healthcare professionals to ensure coordinated care
- Participate in team meetings, in-services, and ongoing training
- Uphold clinical standards and contribute to quality improvement efforts
Requirements
- Strong patient-first mindset with a passion for rehabilitation
- Ability to motivate and engage patients throughout recovery

Qualifications
- Graduate of an accredited Physical Therapy Assistant (PTA) program
- Current PTA license or certification as required by state law
- 1+ year of experience preferred (home health experience a plus)
- Current CPR certification
- Valid driver’s license, reliable transportation, and auto insurance
