Physical Therapist Assistant (PTA) | Home Health

About the role

Provide personalized, home-based physical therapy to patients recovering mobility, strength, and functionality after illness or injury. Collaborate with a multidisciplinary team, including RNs, LPNs, CNAs, and other therapists, under the supervision of a Physical Therapist. Assist in documenting exercises, educating patients and families, and supporting patient goals in their home environment.

Responsibilities

• Deliver tailored physical therapy treatments in patients' homes
• Work closely with a care team to implement the plan of care
• Assist with exercise documentation and patient education
• Support patient progress and safety during therapy sessions

Qualifications

• Graduate of an accredited PTA program with an active Colorado license
• At least one year of PTA experience, preferably in home healthcare
• CPR certification, background check, and drug screening
• Knowledge of home health regulations, clinical judgment, compassion, and strong interpersonal skills
