About the role

Concierge Home Care is seeking a Home Health Physical Therapist Assistant (PTA) to provide one‑on‑one therapeutic care in patients’ homes across Orange Park and Clay County, FL. The position offers a flexible schedule, autonomy, and a pay‑per‑visit model that lets you increase earnings while delivering meaningful care.

Responsibilities

• Provide hands‑on therapeutic care following the plan of care established by the supervising Physical Therapist.
• Assist patients in improving mobility, strength, and independence.
• Collaborate with patients, caregivers, and interdisciplinary teams.
• Monitor patient progress and communicate updates to the supervising PT.
• Complete documentation using streamlined, AI‑assisted tools (WellSky/Kinnser EMR).

Qualifications

• Active, unencumbered Florida Physical Therapist Assistant (PTA) license.
• Valid driver’s license, reliable transportation, and auto insurance.
• Home health experience preferred but not required.

Pay

• Pay‑per‑visit model: $45+ per visit.

Schedule
