Concierge Home Care is hiring a Home Health Physical Therapist Assistant (PTA). Coverage Area: South Brevard County, FL (Territories designed to minimize drive time with clustered scheduling).

Responsibilities

• Provide hands‑on therapeutic care following the plan of care established by the Physical Therapist
• Assist patients in improving mobility, strength, and independence
• Collaborate with patients, caregivers, and interdisciplinary teams
• Monitor patient progress and communicate updates to supervising PT
• Complete documentation using streamlined, AI‑assisted tools

Qualifications

• Active, unencumbered Florida Physical Therapist Assistant (PTA) license
• Valid driver’s license, reliable transportation, and auto insurance
• Home health experience preferred, but not required

Pay

Pay‑per‑visit model: $48–$52 per visit (35 visits per week = full‑time). Average full‑time earnings: $75,000+ annually. Quarterly bonus opportunities. Mileage reimbursement + data plan stipend.
