Responsibilities
- Provide skilled physical therapy treatments under the direction and supervision of a licensed Physical Therapist.
- Implement individualized treatment plans for short-term rehabilitation and long-term care residents.
- Aid residents in improving strength, balance, mobility, endurance, gait, and overall functional independence.
- Monitor resident progress and communicate changes to the supervising Physical Therapist.
- Educate residents and families on therapeutic exercises, mobility techniques, and fall prevention.
- Complete accurate and timely documentation using Net Health.
- Collaborate with nursing staff, physicians, and the interdisciplinary care team to achieve optimal patient outcomes.
- Maintain compliance with all federal, state, and facility regulations.
Qualifications
- Current Physical Therapist Assistant (PTA) license in the Commonwealth of Pennsylvania.
- Graduate of an accredited Physical Therapist Assistant program.
- Skilled Nursing Facility (SNF) or Long-Term Care experience preferred, but new graduates are encouraged to apply.
- Experience with Net Health is a plus.
- Strong communication, clinical, and organizational skills.
- Passion for providing compassionate, resident-centered care.
