Responsibilities

  • Provide skilled physical therapy treatments under the direction and supervision of a licensed Physical Therapist.
  • Implement individualized treatment plans for short-term rehabilitation and long-term care residents.
  • Aid residents in improving strength, balance, mobility, endurance, gait, and overall functional independence.
  • Monitor resident progress and communicate changes to the supervising Physical Therapist.
  • Educate residents and families on therapeutic exercises, mobility techniques, and fall prevention.
  • Complete accurate and timely documentation using Net Health.
  • Collaborate with nursing staff, physicians, and the interdisciplinary care team to achieve optimal patient outcomes.
  • Maintain compliance with all federal, state, and facility regulations.

Qualifications

  • Current Physical Therapist Assistant (PTA) license in the Commonwealth of Pennsylvania.
  • Graduate of an accredited Physical Therapist Assistant program.
  • Skilled Nursing Facility (SNF) or Long-Term Care experience preferred, but new graduates are encouraged to apply.
  • Experience with Net Health is a plus.
  • Strong communication, clinical, and organizational skills.
  • Passion for providing compassionate, resident-centered care.
