About the role

Kettering Home Care provides post-acute care services dedicated to patient growth and family-like support. The role involves skilled therapy services in patients' homes, under the direction of a Physical Therapist.

Responsibilities

• Provide skilled therapy services in patients’ homes under the direction of a Physical Therapist
• Absorb and implement individualized treatment plans to restore independence and improve quality of life
• Support patients with exercises, mobility, strengthening, and functional goals
• Collaborate with interdisciplinary care team members including physicians, therapists, and other professionals
• Document patient progress, changes, and concerns in accordance with agency and Medicare guidelines
• Attend case conferences, trainings, and participate in on-call requirements as needed

Qualifications

• Current Physical Therapist Assistant license in the state of employment
• Minimum of 1 year of PTA experience in an acute care setting
• Preferred home health experience
• Valid driver’s license and auto insurance
• Strong communication, time management, and documentation skills
• Compassionate, dependable, and comfortable working independently in the field
