Physical Therapist Assistant - Home Care

About the Role

Provides physical therapy interventions to patients under the supervision of a Physical Therapist (PT). Assists in monitoring patient progress, documents care, and maintains records according to policy. Follows established treatment plans, modifies procedures within scope, and participates in team meetings and training. Travels to patients' homes or facilities to deliver care, assisting with therapeutic activities, transfers, and use of medical equipment. Supports patient independence and safety, adhering to professional standards and infection control policies. Utilizes technology and supplies for patient care and participates in special projects as assigned.

Requirements

• Licensed as a Physical Therapist Assistant in New York State
• Minimum of two years clinical experience
• Associate’s degree from an approved program or equivalent
• Valid driver’s license may be required

Qualifications

• Bilingual skills preferred
• Basic PC skills preferred
