Responsibilities

• In accordance with PT established Care Plan, provide therapeutic treatment modalities and implement home Physical Therapy including strengthening and ROM exercise, balance, gait, transfer training.
• Provide instruction to patients on topics such as home exercise, home safety, incision care, and medications to achieve patient goals.
• Maintain communication with ordering providers, clinical team, and especially to primary PT to promote progress toward goals or seek direction when needed.
• Participate in quality improvement activities as assigned.
• Timely and accurately document (EMR) all patient assessments, interactions, etc.

Requirements

• Must possess a current, valid Pennsylvania PTA license and have current CPR certification.
• Valid driver's license and ability to travel to case assignments.
