Duties

  • Perform comprehensive patient evaluations and develop individualized plans of care
  • Provide skilled physical therapy services in patients’ homes
  • Conduct Start of Care (SOC), Resumption of Care (ROC), recertifications, and discharge visits
  • Maintain patient progress and adjust treatment plans as needed
  • Educate patients and caregivers on exercises, safety, and ongoing care
  • Maintain timely and accurate clinical documentation in compliance with regulations
  • Collaborate with physicians, nurses, and interdisciplinary team members
  • Ensure compliance with Medicare guidelines and home health regulations

Requirements

  • Education: High school diploma or equivalent
  • Licenses: Current Physical Therapy Assistant license in IL
  • Certifications: CPR certification
  • Years of experience: 1 year
