About the role

Physical Therapist Assistant in home care, providing treatment, exercise instruction, and patient education to improve patient well-being based on a plan of care.

Responsibilities

• Follow state laws and supervise treatment plans set by a licensed Physical Therapist.
• Assist patients with active/passive exercises, gait, ADL, transfer, safety, and prosthetic training.
• Monitor and report patient responses and condition changes to the healthcare team.
• Educate patients, families, and caregivers on therapy techniques within scope of practice.
• Coordinate care with team members and adhere to all safety and procedural guidelines.

Requirements

• Current license, registration, or certification to practice physical therapy assistance in the state.
• CPR certification, valid driver’s license, and reliable transportation.
• Ability to work in diverse home environments and perform physical tasks such as lifting and transferring.
• Availability for weekend rotations.
