About the Role
Provide individualized physical therapy to seniors in skilled nursing and long-term care environments.
Responsibilities
- Implement PT treatment plans to enhance mobility, function, safety, and quality of life.
- Conduct therapeutic exercises, gait and balance training, and functional activities.
- Accurately document patient progress in EMR systems.
- Educate patients and caregivers about therapy techniques and safety.
- Collaborate with an interdisciplinary team in a patient-centered, respectful culture.
Requirements
- Certified Physical Therapist Assistant credential.
- Compassionate, patient-focused approach.
- Strong communication and teamwork skills.
- Ability to follow treatment plans and document accurately.
