Responsibilities

• Providing physical therapy interventions under the direction of a Physical Therapist
• Aiding residents with mobility, balance, strengthening, and functional activities
• Supporting fall prevention and safety education
• Collaborating with interdisciplinary teams to deliver patient-centered care
• Completing documentation in accordance with regulatory and company standards
• Building positive relationships with residents and facility staff

Requirements

• Active Wisconsin Physical Therapist Assistant (PTA) license
• Passion for working with older adults in senior living or skilled nursing settings
• Strong clinical, communication, and documentation skills
• Dependability, adaptability, and a collaborative mindset
• Experience in ALF, SNF, or Memory Care preferred but not required

Qualifications

• Graduate of an accredited school with an associate degree as Physical Therapy Assistant
• Certification by the State of Wisconsin to practice as a Physical Therapy Assistant
