Sierra Home Health in Truth or Consequences is seeking a Physical Therapist Assistant (PTA) to join our compassionate and collaborative home health team. Responsibilities

• Deliver in‑home physical therapy services under the supervision of a licensed Physical Therapist.
• Travel to Truth or Consequences one to two days a week with mileage compensation.
• Carry out treatment plans to help patients regain mobility, function, and independence.
• Communicate progress and changes in patient condition to the supervising therapist.
• Educate patients and caregivers on exercises and safety practices.
• Document visits and progress notes accurately and on time.
• Participate in care team meetings and interdisciplinary planning.
• Uphold infection control and safety standards during all home visits.
• Maintain professional, compassionate rapport with patients and families.

Requirements

• Current licensure as a Physical Therapist Assistant in New Mexico.
• Previous experience in home health preferred, but not required.
• Strong interpersonal and communication skills.
• Confidence working independently in the field.
• Adaptability to changing schedules and home environments.
• Commitment to quality, safety, and patient‑centered care.
• Reliable transportation and a valid driver’s license.
