Responsibilities

• Supports the mission, vision, and values of MHCAH.
• Provides evidence-based, patient-family centered quality care through assessment, diagnosis, outcome identification, planning, implementation, documentation, and evaluation.
• Works closely with patients and families, building better relationships.
• Interacts effectively with patients, families, and health care team members.
• Identifies and initiates appropriate actions based on changes in patients' conditions.
• Operates a vehicle with liability insurance as specified by agency policy.

Requirements

• Graduation from an accredited physical therapy program.
• State of Maine licensure as a Physical Therapist.
• Current, valid driver’s license to operate a motor vehicle.
• Current certification in Basic Life Support for Healthcare providers.
• Minimum one year of full-time equivalent experience in a supervised setting.

Qualifications

• Education: Graduation from an accredited physical therapy program.
• Licenses/Certifications: State of Maine licensure as a Physical Therapist, current, valid driver’s license, and Basic Life Support for Healthcare providers certification.
• Experience: Minimum one year of full-time equivalent experience in a supervised setting.
