Key Responsibilities

• Develop and revise treatment plans with patient and family input
• Provide patient education
• Discharge planning and referrals
• Collaborate with physicians for ongoing evaluation and establish realistic goals
• Document evaluations, treatments, and discharge instructions per departmental guidelines

Qualifications & Skills

• Bachelor's, Master's, or Doctorate in Physical Therapy
• Valid PT license
• BLS certification
• Effective communication, customer service focus, team orientation, and basic computer skills
