Physical Therapist - Acute Care

Responsibilities

• Evaluates And Treats Patients
• Develops and implements plan of care based on evaluation findings and appropriate treatment techniques
• Carries out plan of care with reassessment and changes to plan of care as appropriate
• Maintains/meets the productivity expectations of their department
• Oversees and supervises provision of care by physical therapist assistants
• Plans and helps coordinate discharge plan in collaboration with the multidisciplinary team
• Instructs patient and care giver/family
• Recommends DME required for discharge
• Provides information to providers, nurses, case managers, social workers, and other team members
• Involved in the multidisciplinary patient plan of care and discharge readiness plan
• Documents evaluation, plan of care, progress toward goals, discharge plan/continuity of care for multidisciplinary team, and other involved medical and insurance parties

Education/Mentoring

• Takes an active role in orienting new staff members
• Serves as clinical instructor for affiliating PT and PTA students
• Serves as an advocate for discipline on the multidisciplinary team
• Represents department at problem-solving and committee meetings

Minimum Qualifications

• Bachelor's Degree: Physical Therapy
• LIC - Licensed in Field - State of Ohio
