Responsibilities
- Evaluates And Treats Patients
- Develops and implements plan of care based on evaluation findings and appropriate treatment techniques
- Carries out plan of care with reassessment and changes to plan of care as appropriate
- Maintains/meets the productivity expectations of their department
- Oversees and supervises provision of care by physical therapist assistants
- Plans and helps coordinate discharge plan in collaboration with the multidisciplinary team
- Instructs patient and care giver/family
- Recommends DME required for discharge
- Provides information to providers, nurses, case managers, social workers, and other team members
- Involved in the multidisciplinary patient plan of care and discharge readiness plan
- Documents evaluation, plan of care, progress toward goals, discharge plan/continuity of care for multidisciplinary team, and other involved medical and insurance parties
Education/Mentoring
- Takes an active role in orienting new staff members
- Serves as clinical instructor for affiliating PT and PTA students
- Serves as an advocate for discipline on the multidisciplinary team
- Represents department at problem-solving and committee meetings
Minimum Qualifications
- Bachelor's Degree: Physical Therapy
- LIC - Licensed in Field - State of Ohio
