Physical Therapist - Acute Care

Key Responsibilities

• Develops and revises treatment plans in collaboration with patients and families, providing education, discharge instructions, and community resource referrals.
• Collaborates with physicians to evaluate and establish individualized treatment programs, goals, and discharge plans.
• Documents evaluations, treatment sessions, and discharge information per departmental standards.
• Maintains ethical practices, confidentiality, punctuality, and adheres to dress code policies.
• Communicates effectively with team members, participates in meetings and professional development, and promotes a positive, respectful environment.
• Utilizes resources safely and cost-effectively, observing infection control protocols.
• Supports customer service excellence, applying principles like AIDET, and fosters teamwork and a positive patient experience.

Qualifications

• Bachelor's, Master's, or Doctorate in Physical Therapy.
• Licensure as a Physical Therapist and BLS certification.
• Effective communication, customer service orientation, teamwork, and basic computer skills.
• Additional duties include performing assigned tasks, adhering to policies, and participating in ongoing education to maintain licensure and competencies.
