About the Role

Responsible for delivering and coordinating physical therapy services, including evaluating patients, developing individualized treatment plans, and providing direct therapy in various settings. Ensures patient progress through effective communication with patients, families, and interdisciplinary teams, and maintains thorough documentation of care.

Responsibilities

  • Evaluate patients to gather comprehensive data for treatment planning.
  • Create and implement personalized therapy programs, reviewing plans with patients and instructing caregivers as needed.
  • Utilize functional activities to promote health, adjusting interventions based on ongoing assessments.
  • Provide direct therapy services aligned with patients' physical, emotional, and cognitive needs.
  • Document evaluations, treatment plans, progress notes, and reports accurately.
  • Communicate effectively with the care team and participate in care conferences.
  • Supervise therapy assistants, technicians, and students to ensure safe, effective care.
  • Maintain knowledge of equipment use and safety standards.

Requirements

  • Active state license as a Physical Therapist (required upon hire).
  • Strong communication skills, visual acuity, and ability to work collaboratively.
