Responsibilities

• Perform evaluations and provide skilled physical therapy treatment in a skilled nursing setting.
• Develop individualized care plans to improve mobility, strength, balance, and overall function.
• Collaborate with the interdisciplinary team to ensure continuity of care.
• Complete documentation in accordance with regulatory and company standards.

Requirements

• Active Wisconsin Physical Therapist license.
• Strong clinical reasoning, communication, and organizational skills.
• A passion for working with older adults in long-term care environments.
• Dependability, flexibility, and a team-oriented mindset.

Qualifications

• Graduate of an accredited school with a bachelors, master’s, or doctoral degree in Physical Therapy.
• Licensed as required by the State of Wisconsin to practice as a Physical Therapist.
