About the role
Join Boone Memorial HomeCare, part of LHC Group and the Optum family of businesses, as a Physical Therapist in Home Health. You’ll assess and evaluate patient care needs related to functional status, mobility, endurance, pain, and other medical conditions. Based on your findings, you’ll develop and implement a treatment plan to improve and enhance the patient’s well-being, then evaluate progress. Service area: Boone and Lincoln Counties.
Responsibilities
- Evaluate the patient’s functional status and physical therapy needs
- Assist the physician in developing the therapy plan of care
- Provide services within the scope of practice as defined by state laws, in accordance with the plan of care, and in coordination with the health care team
- Observe and report the patient’s response to treatment and any changes in condition to the supervising nurse and/or physician
- Treat patients to relieve pain, develop or restore function, and maintain maximum functioning using acceptable standards of practice
- Instruct the health care team, patient, and family/caregivers on the plan of care, functional limitations, and progress toward goals
- Visit patients according to the Plan of Treatment, complete a progress note for each visit, and submit progress notes per policy
- Complete all patient evaluations and develop the PT plan of care within state-specific guidelines
- Report outcomes of evaluation, goals, and anticipated projected frequency of care
Requirements
- Current Physical Therapy licensure in West Virginia
- Current driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation
- Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
Preferred Qualifications
- Current CPR certification
Pay
Annual target pay range: $82,048 – $123,072. Compensation includes two components:
- Per-visit point pay: $47.34 – $71.00 per productivity point (points vary by visit type and include direct and indirect care activities)
- Hourly pay: $39.45 – $59.17 for non-visit activities such as orientation
Total cash compensation assumes full-time employment (40 hours/week) at full productivity and may increase over time based on performance and local labor markets.
