About the Role

Now hiring Physical Therapists to cover the South Bend area. At Saint Joseph VNA Home Care, we believe care is best delivered where people feel most comfortable—at home. This role offers one-on-one care in patients' homes, allowing you to build real relationships while using cutting-edge technology to help patients stay safe and healthy.

Responsibilities

• Provide one-on-one physical therapy care in patients' homes.
• Maintain clinical practice skills and adhere to organization’s policies and procedures.
• Accept personal responsibility for maintaining professional relationships with agency’s clients and families.
• Function in an environment with minimal direct supervision.
• Learn and adhere to conditions of participation and home healthcare practices.

Requirements

• Possession of a degree, including an internship, from a baccalaureate/university program in physical therapy approved by the American Physical Therapy Association.
• Licensed to practice as a physical therapist in the state of practice.
• Minimum of one (1) year of experience as a physical therapist or completion of a preceptor program for a minimum of 6-8 weeks.
• Current working knowledge of conditions of participation and home healthcare experience preferred.
• Current Driver’s license and reliable transportation to and from work site.
• Ability to demonstrate commitment to the mission and Organizational Code of Ethics, and adhere to the Compliance Program.
